Role Description
The Clinical Operations Associate will join Zócalo Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing, and Zócalo Health will lead the transition from fee-for-service to value-based care for the growing Latino community in the U.S.
An integral member of the Zócalo Health founding team, you will be responsible for day-to-day clinical and operational workflows across Community Supports (CS), Enhanced Care Management (ECM) and Community Health Worker (CHW) programs, with end-to-end ownership of authorizations and referrals. This role ensures operational and clinical excellence, compliance, and alignment between administrative, clinical, and service delivery teams. Responsibilities include working cross-functionally with ECM, CS, and clinical operations leaders to optimize referral and authorization workflows. Supervisory responsibilities may include CS PSS – Outreach to ensure workflow alignment.
This position reports to the Director of Clinical Operations.
The Clinical Operations Associate will contribute in the following ways:
- Submit, track, and manage all Community Supports, Enhanced Care Management and Community Health Worker authorizations/reauthorizations/referrals
- Update Athena with authorization information
- For Community Supports: add members to the outreach assignment tracker and ensure compliance with the 24-hour assignment standard
- Ensure workflow alignment with referral/authorization processes
- Conduct internal checks for compliance, quality assurance, and clinical alignment
- Troubleshoot operational barriers and streamline workflows across ECM and CS
- Support reporting, program performance metrics, and data-driven decision-making
- Collaborate cross-functionally with ECM, CS, and clinical operations leaders to optimize processes
Qualifications
- 2 – 5+ years in healthcare operations, care management ops, or clinical admin roles
- Hands-on experience with authorizations, reauthorizations, and referrals in Medicaid/Medi-Cal, and value-based care or managed care environments
- Familiarity with: Medi-Cal ECM & Community Supports, including Health plan requirements, timelines, and documentation standards
- Proficiency in maintaining logs, dashboards, trackers, and other data metrics, including EHR systems.
- An understanding of working with data for: Performance metrics, Operational reporting, and process improvement
