Licensed Vocational Nurse (LVN) – All Shifts

Bakersfield Post Acute
Pay Range: $28–$30 per hour (BOE)

Position Summary

Bakersfield Post Acute is seeking compassionate and skilled Licensed Vocational Nurses (LVNs) for all shifts. The primary purpose of this position is to provide direct nursing care to residents and supervise the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs).

The LVN ensures care is delivered in accordance with federal, state, and local regulations, facility policies, and direction from the Director of Nursing or Nurse Supervisor to maintain the highest level of quality resident care.

Essential Duties and Responsibilities

Administrative Functions

  • Direct the daily functions of nursing assistants in accordance with facility policies and regulatory guidelines.

  • Ensure nursing personnel comply with established procedures and standards of care.

  • Provide written and oral reports regarding shift activities.

  • Participate in quality assurance programs and facility surveys.

  • Admit, transfer, and discharge residents as required.

  • Complete incident/accident reports and required administrative documentation.

  • Maintain daily census reports and forward required documentation to appropriate departments.

  • Protect resident confidentiality and comply with HIPAA and facility information security policies.

Charting and Documentation

  • Complete admission, transfer, and discharge documentation accurately and timely.

  • Receive and transcribe physician orders in accordance with policy.

  • Document nurses’ notes clearly, accurately, and descriptively.

  • Record diet changes and communicate with Dietary Services.

  • Ensure all chart entries are signed and dated appropriately.

  • Report discrepancies in physician orders or documentation to the Nurse Supervisor.

Medication Administration

  • Prepare and administer medications as ordered by the physician.

  • Verify resident identity prior to medication administration.

  • Ensure proper narcotic documentation and reporting of discrepancies.

  • Monitor medication records for completeness and compliance with stop-order policies.

  • Maintain adequate medication and supply inventory for assigned shift.

  • Dispose of medications and narcotics per facility policy.

Personnel and Leadership

  • Provide leadership and supervision to assigned nursing staff.

  • Conduct shift rounds to ensure compliance with care standards.

  • Report staffing concerns to the Nurse Supervisor.

  • Maintain a professional and collaborative working relationship with interdisciplinary departments.

  • Participate in staff development and in-service training programs.

Nursing Care Functions

  • Review resident care plans daily and ensure implementation.

  • Monitor resident conditions and notify physicians and families of changes.

  • Perform skilled nursing procedures including wound care, catheterization, tube feedings, suctioning, and specimen collection.

  • Take and record vital signs as required.

  • Ensure residents receive care in accordance with individualized care plans.

  • Participate in discharge planning and coordination of services.

  • Maintain a calm, respectful, and supportive environment for residents and families.

Supervisory Responsibilities

As an LVN, you are delegated the authority, responsibility, and accountability necessary to carry out assigned duties and supervise nursing assistants during your shift.

Qualifications

Education and Experience

  • Graduate of an approved LVN program.

  • Current, active, and unencumbered LVN license in the State of California.

  • Experience in a skilled nursing facility preferred.

Required Skills and Abilities

  • Strong knowledge of nursing practices, procedures, and regulatory guidelines.

  • Ability to make independent decisions and exercise sound clinical judgment.

  • Ability to work effectively with residents, families, physicians, and staff.

  • Leadership skills and the ability to supervise assigned personnel.

  • Proficiency in electronic medical records; PCC experience preferred.

Certificates and Licensure

  • Current LVN license in good standing.

  • Current CPR certification.

  • Must maintain required continuing education.
