Shift and Status

Full time, 8:00 a.m. to 4:30 p.m., No weekends

Hybrid: Home and Facility Visits, travel to patients' homes, nursing facilities, and hospitals to provide direct care in Saint Joseph, Stevensville, Benton Harbor surrounding areas.

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. We provide both inpatient and outpatient medical services throughout Southwest Michigan.

Scope of Work

Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and

delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing

individualized care plans, and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.

Qualifications

Required

• Associate’s degree in nursing or equivalent
• 2 year of relevant experience preferably in Hospice
• Michigan Registered Nurse (RN) license upon hire
• State of Michigan Driver’s license upon hire

Preferred

• Bachelor’s degree
• 1 year of relevant experience in Hospice
