About the role

Exciting Opportunity for a Full-time LCSW in NYNuvance Health has multiple exciting opportunity for an experienced LCSW in Highland, NY. The Clinical Care Coordinator known in the PC offices as a Behavioral Health Consultant is an integrated member of the Primary Care team and provides behavioral health services to the primary care patients. The schedule work hours are flexible within the Primary Care office hours Monday through Friday. 

Summary:

Provide behavioral health consultation to children, adolescents, adults and families with behavioral health needs with the goal of improving psychosocial functioning of Health Quest Primary Care patients. Performs assessments and formulates treatment plan and or discharge/transition plans in collaboration with patient, provider, and possible family and multidisciplinary healthcare team. Provides ongoing monitoring and evaluation of plans to meet the individual needs of patients throughout the care continuum. Assists in resource management through facilitation of care and participation in program planning, data collection and analysis. Provides education to patients/families regarding coverage, care options and community resources.

Other Information:

Education  Essential: * Master's Level Degree. 

Minimum Experience: three to five years.

* Licensed Clinical Social Worker in NY and CT.

The candidate must have: Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions; Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; Comfort with the pace of primary care and working with an interdisciplinary team; Excellent communication skills; Knowledge of psycho-pharmacology; Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.

Broad based clinical background and hospital case-management or Crisis Intervention experience, behavioral health community experience, medical social work experience, outpatient treatment, SUD experience, overall experience across the continuum of behavioral health care settings. Excellent oral and written communication skills. Basic computer skills. Prioritize the daily workload and utilize tools dedicated to the position/function. Deal with a high degree of public and patient contact and interaction, and make independent decisions, perform concurrent tasks, manage stressful situations and balance multiple priorities.
