About the Role
The PRN Clinical Navigator (CN) is a licensed Doctor of Physical Therapy (DPT) responsible for providing flexible, on-demand clinical support to our core Clinical Navigation team. This role is designed to create elastic capacity, ensuring high-quality patient engagement during demand surges, staff absences, or seasonal spikes.
As a PRN Clinical Navigator, you will utilize your clinical expertise to manage specific, high-impact tasks within the patient journey, such as conducting initial assessments and patient education, and/or longitudinal engagement through a patient’s care journey with TailorCare.
Responsibilities
The PRN Clinical Navigator operates within a clearly defined scope to supplement the core clinical navigation team during activated "triggers," such as high volume or PTO coverage.
Clinical Intake & Assessment
- Manage and process patients in the pending queue to reduce wait times and prevent bottlenecks.
- Conduct clinical assessments to determine patient needs and establish appropriate clinical pathways.
- Conduct patient education through cadenced engagement to set expectations for the program and ensure adherence, as well as patient functional improvement.
Documentation & Quality
- Complete all clinical documentation within established CN workflows and standardized decision trees.
- Adhere to departmental quality oversight, including QA audits and feedback loops with CN Leads.
- Identify and utilize clear escalation pathways for complex clinical cases.
Schedule & Commitment
- General Availability: 2–3 days per week (approximately 2–4 hours per day) for overflow of patient needs; however, must remain flexible.
- Coverage Support: 6–8 hour shifts as needed for planned PTO, onboarding, or training gaps; however, could be less based on patient demands.
- Activation: Support is activated by Clinical Ops leadership based on demand thresholds or staffing transitions.
Qualifications
Required:
- Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT)
- Current, unrestricted PT licenses in Georgia, Texas and/or Colorado (Compact License preferred)
- Willingness to obtain additional licensure in new states based on TailorCare’s market expansion
- Minimum 3 years of relevant experience in physical therapy or MSK-related clinical roles, with demonstrated leadership or mentorship experience
- Prior experience in digital health care navigation environments or telemedicine care delivery
- Expertise in motivational interviewing, behavior change coaching, and empathetic communication
- Strong written and verbal communication skills tailored to clinical and non-clinical audiences
- Experience using data to optimize clinical workflows and identify opportunities for improved patient outcomes
- Comfortable working in dynamic, high-growth, tech-enabled environments
- High comfort level with technology, including Google Suite, CRMs, dual-monitor workflows, and real-time communication platforms
Preferred:
- Prior exposure to managed care, commercial and medicare advantage populations, or value-based care models
- Spanish-speaking preferred
