United Health Services is seeking a Physical Therapist to join our compassionate and collaborative Home Care team. In this role, you’ll provide one-on-one therapy to patients in their homes—helping them regain strength, improve mobility, and maintain independence in a familiar environment. Compensation Range:

$33.57 - $51.39 per hour, depending on experience

Job Responsibilities

  • Assess patient strengths, limitations, and needs using standardized measurement, prediction, and evaluation tests

  • Develop and implement individualized short- and long-term treatment goals based on patient assessments

  • Administer and interpret clinical physical therapy services, adjusting treatment plans as needed based on patient response

  • Maintain accurate and timely patient treatment records, documenting progress and changes in electronic health systems

  • Ensure all documentation meets requirements of reimbursement and regulatory agencies, completed within two business days

  • Establish and maintain effective working relationships with patients, families, and interdisciplinary team members

  • Participate in patient care conferences and collaborate with other healthcare professionals for coordinated care

  • Provide education to patients and families regarding treatment plans, safety, and self-care techniques

  • Participate in discharge planning and make appropriate referrals, coordinating with vendors for durable medical equipment

  • Supervise and provide direction to Physical Therapist Assistants during treatment sessions

  • Maintain licensure requirements and pursue continuing education to stay current with professional standards

  • Engage in departmental and organizational performance improvement initiatives to enhance quality of care

Position Qualifications

Minimum Required:

  • Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York

  • Graduate of an accredited school of physical therapy with a Bachelor's or Master’s Degree or equivalent in physical therapy

  • Current CPR certification

  • Previous experience in a licensed healthcare setting

Preferred:

  • Master’s or Doctorate Degree in Physical Therapy

  • Previous physical therapy experience in a hospital-based setting
