Clinical Pharmacist PRN Rotating Position Summary

Decentralized unit-based Clinical Pharmacists are valued team members working in patient care areas optimizing patient medication therapies by monitoring drug selection, drug dosage, frequency, food/drug allergies, drug- drug interactions and duplication, medication reconciliation and discharge medications.
They are a resource for drug information and education for physicians, nurses, and patients. Also, dose medications under protocols.
They also round with Intensivists and Hospitalists playing an active role making recommendations for therapies and changes in therapies.
They are ACLS certified responding to all Code Blues and RRTs.
 

THE CLINICAL PHARMACIST PRN ROTATING CANDIDATE WILL POSSESS THE FOLLOWING.

Minimum Education: A Bachelor Degree in Pharmacy with 5 years Clinical Experience or Pharm. D. is required.

Minimum Experience: 5 years Clinical Experience for BSc. or Pharm. D. Rotations.

Required Certifications/Registrations/Licenses: Licensed as Pharmacist in S.C. BLS and ACLS certification required."

This individual’s responsibilities include (but are not limited to): Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records. Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions. Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained. Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions. Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc. Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. Provide education to patients on the use of medications as needed. Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use. Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed. Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.

Education:

Required: Pharm.
D. or BS in Pharmacy

Experience:

Preferred: Hospital experience

Certifications:

Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities.
Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.

Physical Demands:

Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position.
