JOB TITLE:   Medical Director – Care Coordination 

SHIFT: 32 Hours Bi-weekly, Part-time 0.4 FTE


Summary/Objective

 Under general direction of the CEO responsible for supporting the development of strategic planning for service line care delivery and coordination in collaboration with service line leaders. This position should ensure high standard of patient care and satisfaction. 


Essential Functions

  

  • Assist in developing and coordinating an integrated health care model.
  • Lead and be a resource for service line process improvement projects: standardization of processes, improved outcomes, and cost reduction.
  • Support the development of strategic planning for service lines in collaboration with service line leaders.
  • In conjunction with CMCH leadership and medical staff create pathways and/or protocols that organize patient care activities to achieve safer and more effective care.
  • Work with medical staff and other stakeholders to evaluate care transitions and effectiveness on quality and outcomes on a routine basis. 
  • Navigate complex and dynamic changes in the regulatory landscape of population health.
  • Incorporate Patient Experience into design of care pathways.
  • Analyze performance data and make informed recommendations to leadership to enhance service delivery.
  • Collaborate with stakeholders to evaluate policies procedures and processes for care coordination and support associates to reflect impactful changes.
  • Work closely with medical staff, nursing, and leadership to develop, implement, and review the department’s service goals.
  • Assess referral source communication pathways and continuum of care pathways and work with system leadership to optimize and improve. 
  • Identify opportunities to refer patients in our care to preferred and strategic partners based on clinical outcomes and optimal transitions of care.
  • Provide gap analyses for identified service deficits. 
  • Collaboration with the healthcare team to assess and evaluate where changes need to be made with processes.
  • Research, receive ongoing training and collaborate with professionals to stay up to date with advancement and developments in the field, integrating them, where applicable, into service line planning.
  • Mentoring of service line leadership by providing feedback, coaching and support
  • Performs other duties as assigned.

Qualifications

  • MD or DO

Experience

  • Experienced Physician, minimum 10 years practice experience (especially hospital-based practice experience). 
  • Executive healthcare experience within a complex healthcare organization, including acute and ambulatory care.

Skills/Competencies

  • Excellent written and oral communication skills
  • Ability to work with teams and individuals, grounded in evidence-based practice, in a professional and friendly manner. 
  • Knowledge of complex hospital and healthcare systems, processes, and quality/performance improvement
  • Solid understanding of hospital throughput and designations of inpatient status, observation service, and extended recovery.
  • An understanding of medical protocols and criteria and how they impact status determination.
  • Understanding of clinical protocols, reimbursement challenges, and managed care principles
  • Ability to perform as a Physician Advisor to ensure the medical management of cases and quality of patient care.
  • Working knowledge of Electronic Health Records (EHR), experience with excel and clinical documentation.
