  • Min: $109,762

  • Mid: $137,155 (~10 years of experience)

  • Max: $150,000 (15+ years of experience)

Overview

A large Level I Trauma acute care hospital in Central Florida is seeking an experienced Director of Case Management to lead and oversee hospital-wide care coordination and discharge planning operations.

This is a senior leadership role responsible for clinical, operational, and regulatory oversight of case management services in a 204-bed hospital with high patient volume. The Director will lead a multidisciplinary team and partner closely with executive leadership to ensure quality outcomes, compliance, and efficient utilization of resources.

Responsibilities

  • Provide strategic and operational leadership for all case management functions

  • Direct daily department operations, including staffing models, productivity, onboarding, and competency management

  • Ensure effective care coordination and discharge planning processes across all service lines

  • Monitor department performance through data analysis, quality metrics, and regulatory benchmarks

  • Lead performance improvement initiatives to enhance patient outcomes and throughput

  • Ensure compliance with federal, state, and accreditation standards

  • Collaborate with executive and physician leadership, including finance, nursing, medical staff, ethics, and legal partners

  • Partner with division leadership to achieve financial, quality, and operational goals

  • Foster a culture of accountability, collaboration, and professional development

Qualifications

Required:

  • Bachelor’s degree

  • Active RN, LCSW, or LMSW license

  • Minimum 3 years of acute care hospital case management experience

  • Minimum 2 years of case management leadership experience in an acute care setting

  • Recent acute care experience (within the past year)

Preferred:

  • Master’s degree in Nursing, Healthcare Administration, or Business Administration

  • Experience leading case management teams in large or high-volume hospitals

  • Strong regulatory, utilization review, and discharge planning expertise
