The Registered Nurse Clinical Program Coordinator for Chronic Diseases is responsible for the coordination and implementation of comprehensive care programs for patients with chronic illnesses. This role provides close oversight and support for implementation for clinical programmatic development; including involves developing care plans, collaborating with multidisciplinary teams, monitoring patient progress, and providing education and support to patients and their families.


Key Responsibilities: 

  • Program Development and Coordination:
    • Design and manage clinical programs tailored to the needs of patients with chronic diseases within primary care and health care organizations.
    • Ensure the programs are evidence-based and aligned with best practices.
  • Education and Support:
    • Provide education and ongoing support to providers and nurses who care for patients with chronic diseases.
  • Team Collaboration:
    • Work closely with internal and external multidisciplinary teams, including doctors, specialists, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care aligned with system initiatives.
  • Quality Improvement:
    • Participate in quality improvement initiatives to enhance the effectiveness and efficiency of chronic disease management programs.
  • Documentation and Reporting:
    • Maintain accurate and up-to-date quality reports and program documentation. Ensure compliance with regulatory requirements and standards.
  • Community Outreach:
    • Engage with community organizations and resources to support patient care and program initiatives. Facilitate access to additional services and support as needed.
  • Quality and Data Review:
    • Conduct regular quality audits and data reviews to ensure compliance with clinical guidelines and performance standards.
    • Analyze data to identify trends and areas for improvement.

Qualifications: 

  • Education: Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing or related field preferred.
  • Licensure: Current, unrestricted Registered Nurse (RN) licensure in the state of practice. (Obtain additional state licensure as required)
  • Experience: Minimum of 3-5 years of clinical nursing experience, with a focus on chronic disease management.
  • Experience: 2-3 years program coordination, quality reporting, program implementation or leadership role preferred.
  • Skills: Excellent communication, organizational, and leadership skills. Ability to work collaboratively with a multidisciplinary team. Proficient in electronic health records (EHR) and other relevant software.
  • Certifications: Certification in chronic care management or related specialty preferred.
