We are seeking a Community Health Specialist 2 (CHS2) with a desire and experience working with the to provide field services/case management to program participants, including connecting participants to social support services, advocacy, peer support, and attending primary care and other appointments with clients. You will work alongside the Transition and Community Services Program to support health education and outreach, testing, vaccinations and supporting clients transition safely back into the community. 

As a Community Health Specialist 2 in Corrections Health, you will help serve in our three Correctional Health facilities as well as the Mead Building .
