Snowline Hospice is a nonprofit, community-based organization dedicated to meeting the unique physical, emotional, and spiritual needs of those who are nearing the end of life's journey. Our mission at Snowline is serving our community by supporting patients near end of life and guiding loved ones through grief. Our nursing team is dynamic and supportive; not only caring for the patients and friends and families we serve, but for each other, every day.
 

Here's how you'll be caring.

Support patients living life to the fullest in the time they have left.

Work collaboratively with an interdisciplinary team including, other registered nurses, LVNs, social workers, spiritual care providers, and home health aides as well as bereavement specialists, physicians, and volunteers.


Each day. Essential functions.

  • Respond and collaborate on incoming phone calls, texts, or emails
  • Assess patient needs via phone and aid in triage and prioritizing patient visits
  • Establish rapport with patients and families over the phone
  • Coordinate visiting nurses to patients location based on need
  • Calm, soothe and empathize with the strong emotions our patients and families are experiencing
  • Motivate and encourage a team of healthcare professionals in providing the best care possible to each and every patient
  • Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
  • Assist with the management of clinical notes, health records, and systems to ensure appropriate and accurate data

Requirements

  

Our ideal candidate. 

  • BSN, Associate Nursing Degree, or a diploma from an accredited school of nursing
  • Valid California Registered Nursing or LVN License
  • Minimum one year experience in nursing (one year hospice experience preferred)
  • Self-directed and able to work with minimal supervision, ability to organize and prioritize workload independently
  • Excellent observation, problem solving, verbal and written communication skills
  • Demonstrated ability to efficiently use a computer, prior electronic medical record (EMR) experience preferred
  • Current BLS/CPR certification
  • Valid CA driver’s license, with a reliable vehicle that is insured in accordance with state requirements or willingness to participate in our generous car lease program
  • Is open to occasionally working evenings or weekends 
  • Can work remote and is also available to be on-site occasionally and can travel throughout our territory on occasion
