Job Description

Southwestern Behavioral Healthcare is currently seeking a full-time Case Manager to add to our team of professionals.  This position will work in our Community Support Services department providing Assertive Community Treatment services to clients assigned to the team.

WHAT IS THIS POSITION RESPONSIBLE FOR?

  • Delivery community-based case management and skills training services to adult clients, guided by recovery and person-centered principles. 
  • Conduct assessments (including the ANSA tool) to evaluate psychiatric stability, daily functioning, strengths, limitations, and environmental needs.
  • Translate assessment results into actionable goals in a Person-Centered Treatment Plan.
  • Provide individualized skills training and development based on client and family needs.
  • Participate in rotating Saturday on-call coverage within the Community Support Services department.
  • After completing the provisional employment period, join the weekend and holiday coverage rotation.
  • Maintain compliance with efficiency standards, including chart documentation and client contact benchmarks.
  • Work overtime when authorized to meet client needs.

OTHER DUTIES:

  • Stay informed about community resources and entitlement programs.
  • Monitor client progress, use of services, and overall safety.
  • Represent Southwestern with professionalism in conduct, communication, and appearance.
  • Adhere to all organizational policies and procedures.

Job Qualifications

WHAT'S REQUIRED FOR THIS POSITION?

  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field.
  • Knowledge of available community resources is preferred.
  • Previous work with the SMI population is highly preferred.
  • Demonstrated proficiency with interpersonal and writing skills.
  • Excellent communication, time management, and organizational skills.
  • Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance.
  • Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.
