Job Description: This position conducts home visits to assess client needs at the time of initial intake and on-going supervisory visits when needed. Develops client care plans detailing activities to be performed in the home by the Assisting Hands caregivers.   Reports To: Administrator

Qualifications:
  • Graduate of an accredited nursing school.
  • Licensed by the State of Utah as a Registered Nurse, in good standings and responsible for applying the minimum standards of nursing regardless of the practice setting.
  • Has a minimum of 1 year nursing experience, some experience in a home care setting preferred.
  • Must have effective verbal and written communication skills with clients, staff and management.
  • Has substantial specialized judgment and skill based on knowledge and application of the principles of biological, physical and social science in nursing.
  • Demonstrates knowledge of Utah Board of Nursing Rules and Regulations related to Delegation and Tasks Not Requiring Delegation.
  • Is experienced in Conflict Resolution as outlined by the Utah Board of Nursing in the Practice-Delegation Resource Packet.
  • Able to perform duties and responsibilities with or without reasonable accommodation.
  • Must be able to represent Assisting Hands Home Care well in the community.
  • Must have a reliable car and be willing to drive throughout the service area as needed. 

Primary Duties: 1. Completes all initial assessments for new clients and develops a Client Care Plan to detail all daily activities to be performed in the                   home by caregivers.  2. Meets with clients to explain and ensure understanding of assessment decisions, tasks to be performed and complete written & signed          agreements.3. Determines and documents that the client’s needs do not exceed the level of personal care services within the scope of Assisting Hands.4. Should a client’s needs exceed personal care services, Assisting Hands shall make a referral to a licensed health care professional or              appropriate alternate service.  5. The RN will be responsible for performing a new assessment and drafting an updated Care Plan at any time during service that there is a         change in client condition.6. Assists the Administrator, as needed, regarding decisions to accept clients for services or recommend other more appropriate placement.7. Collaborates with the Administrator and caregiver supervisor to ensure that all client needs are being met.8. Communicates with the client’s healthcare team, as needed, to ensure care and services for the client are coordinated.
Hours: Part-Time, 1-2 days per week. Most visits will be Monday-Friday, between the hours of 8:00-5:00. Occasional request for after hours or weekend visits may be needed in special cases. Hours are flexible.
Pay: $40/Per Hour
This position offers the flexibility of W-2 or 1099 employment, allowing you to choose the classification that best suits your needs and preferences.   Compensation: $40.00 per hour
