Position Overview:Join our dedicated team as a Director Of Nursing  in Home Health Care, providing high-quality, compassionate care while ensuring client and caregiver compliance with agency standards. This is a rewarding opportunity to work in a flexible, remote setting, with responsibilities that include conducting assessments, monitoring patient progress, and supporting the overall care coordination of our clients in New Jersey. 
Experience:  - Preferred: Previous experience in Home Health Care with a proven ability to deliver exceptional care while ensuring client and caregiver compliance with care plans and agency protocols. Key Responsibilities:    - Conduct comprehensive assessments of both clients and caregivers, ensuring adherence to agency policies and regulatory requirements.    - Review and update care plans based on the findings of these assessments.     - Initial In-Home Client Assessment: Perform an in-home assessment to establish a personalized plan of care.    - 30-Day Phone Call Assessments: Conduct follow-up phone assessments to monitor patient progress, address concerns, and ensure continued compliance.    - 60-Day In-Home Client Assessments: Reassess clients in their homes to evaluate health status, care plan effectiveness, and overall patient well-being.     - Consistently uphold and implement the agency's nursing care standards, policies, and procedures to ensure safe and effective patient care.     - Collaborate seamlessly with the healthcare team, caregivers, and agency staff, ensuring clear communication and continuity of care.    - Establish a positive rapport with clients and caregivers, providing education and support to help them navigate the home health care process.     - Ability to communicate clearly and professionally in English, ensuring effective interactions with clients, caregivers, and team members     - Perform any additional tasks or functions as required to ensure agency compliance with healthcare regulations and maintain high standards of patient care

Compensation: $70.00 - $90.00 per day
