The ISCH staff will provide Individual Support to Massachusetts Waiver Contracted Individuals – regular or intermittent services designed to develop, maintain, and/or maximize the participant’s independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills, to achieve objectives of improved health and welfare and to the support the ability of the participant to establish and maintain a residence and live in the community.

Responsibilities:

The ISCH Worker accepts responsibility for his/her own actions and:

  • To empower participant to make informed contribution towards any decision about their overall independence.
  • To assist Participants with booking, tracking, and attending medical appointment as needed
  • Assist with transportation scheduling and/or transporting individuals in their own vehicle.
  • Educating participant on basic budgeting skills and financial management
  • Empower and assist participants to build neighborhood relations and community engagement.
  • Assist with benefits applications such as SNAP and documentation renewals as needed.
  • To work with participants on goals and objectives set in the individual service plan.
  • Completes required documentation on generations accurately and on a timely basis.
  • Assist with attaining community resources needed to enhance overall independence.
  • Attends agency orientation, in-service education programs and staff meetings as required. 
  • To be familiar and work within the policies and procedures of the service
  • Utilize effective time management techniques to ensure tasks are prioritized, and agreed deadlines are met.
  • To undertake other duties commensurate with this post.
Qualifications:
  • Minimum of high school diploma or advance human service-related field preferred.
  • Experience with ABI or TBI clients or Prior home care experience preferred.
  • Knowledge on person cantered care is a plus but not necessary.
  • Ability to research community resources or prior knowledge of local community support system. 
  • Have good IT skills, including electronic documentation and communication.
  • Have good listening and communication skill.
  • Be able to travel around the borough independently, preferably by car.
  • Be flexible around working hours and be prepared to work some evenings or weekends as reasonably required by this role.
  • Be impartial and non-judgmental.
  • Committed to the rights of seniors and disabled people to live dignified lives.
  • Is flexible, sensitive to the needs of others, reliable, and able to cope with emergencies.
