This is a highly independent and responsible Advanced Practice Registered Nurse position with the HIV/Specialty Care Clinic. This position is part of a multidisciplinary team that ensure proper care and treatment to clients who are seen for the following services: HIV/AIDS, PrEP, nPEP, and Community Outreach.
Duties and Responsibilities
This is highly confidential and specialized position dedicated to providing services for persons living with HIV/AIDS. This dedicated care position is part of a team that are providing enhanced care coordination services to persons living with HIV in Seminole County. Main goals are to increase the numbers of persons living with HIV/AIDS who engage and remain engaged in HIV primary-specialty care, increase the number of persons maintain viral load suppression. The position incumbent will be highly skilled in the performance of clerical and administrative duties and will also work independently gathering data, preparing reports, and providing day-to-day operational assistance to the Program Manager, and other staff as needed.
Apply advanced nursing principles and techniques in the care and treatment of clients, prescribe, administer and documents medications ensuring the Rights of Medication Administration, be sensitive when interviewing clients about a sensitive communicable disease issue such as HIV/AIDS or sexually transmitted disease, utilize advanced nursing protocols in the care of clients. Provide a holistic care that may include diagnosing and treating sexually transmitted infections, hepatitis, tuberculosis, conduct initial/annual physicals, well woman health exams (including pap smear, pelvic exam, & breast exam when warranted), and counseling.
Make appropriate referrals as indicated, evaluates immunization status, and administers immunizations per protocol using correct techniques, log administered vaccines into Florida Shots, Health Management System (HMS) and Care Ware – Provide Enterprise System, enter medication refill requests into HMS – Emdeon, accurately documents all services provided.
Review lab results and provide follow up to those assigned as abnormal or priority especially viral loads and CD4 counts. Assess client’s well-being including but not limited: medication adherence, reasons for missed appointments, housing conditions, transportation, provide and implement clinical recommendations.
Provide care coordination especially to newly diagnosed people living with HIV/AIDS. Serve as liaison with case management team, specialty care providers/partners and State Telehealth Providers. Reviews and authorizes prescription requests made by pharmaceutical providers and clients according to treatment needed by each client. Provides education to clients, partners, team members and community.
Assists Program Manager and/or designee with the coordination of pharmacy services for approved clients as well as with additional areas including pilot studies, surveys, focus groups, reports, etc. Maintains 100 % compliance with confidentiality protocols as well as security and integrity as determined by the Florida Department of Health HIV/AIDS Section Bureau of Communicable Diseases, as well as all other databases, including Medicaid (FLMMIS), Health Management System (HMS), Availity, Change Revenue (Optum), STARS, Florida Shots as assigned. Reports immediately to the Manager or designee of any problems/complaint’s clients report, as the needs of the client’s mandate.
Enter all client encounters – both face to face and by phone into the Health Management System (HMS) and Provide Enterprise System for tracking and reporting purposes. Submits required reports, maintains assigned records, completes/returns all paperwork pertaining to job duties accurately, legibly and in proper order.
