Facilitator II: Master's degree in Mental Health, Human Services, closely related Field,
Facilitator I: Bachelor's degree in Mental Health, Human Services, or closely related Field.
Facilitator II: Master’s degree: One (1) year experience in mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high risk children and families.
Facilitator I: Bachelor’s degree: Two (2) years’ experience in mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high risk children and families.
Must possess a valid CA driver's license and maintain a driving record that meets the company driver eligibility policy.
How you will make a difference:
This position has primary responsibility for coordinating Intensive Home-Based Services (IHBS), Therapeutic Behavioral Services (TBS), and Therapeutic Foster Care (TFC) Services , including engagement, Child and Family Team (CFT) formation and implementation, development of the CFT Meeting Action, and case management services to children, adolescents and their families. When indicated as a need by the youth/family, the Facilitator may provide mental health services such as individual rehabilitation and coaching. He/she acts as a liaison with external agencies and convenes the Child and Family Team. The Facilitator takes the lead to ensure the team consists of family members, County representatives, the client, and others selected by the family. The Facilitator works to ensure all team members are active participants in the CFT Process. The facilitator works with family to generate Vision/Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs. Collaborates with team members to ensure adherence to program process and principles.

Community Based programs.
Provides programs for children, adolescents, and transitional age youth (TAY) with Mental Health services.
An experienced staff of psychiatrists, psychologists, social workers, mental health therapist / Clinician, parent partners, and other counselors
Provide case management, mental health care, and a range of support services in schools, homes or other community settings.
