Schedule Details/Additional Information:

Flexibility for all shifts. A minimum of one night shift a week. Also, will be expected to have flexibility to help with patient coverage.

Pay Range

$21.45 - $32.20

• Monitors TOWs in order to help the team meet their individual and team goals.

• Creates a healthy and motivating work environment and atmosphere.
  Creates workflows/procedures with external and internal stakeholders
  Oversees day to day operation and performance of TOWs.
  Provides patient outreach and resource navigation.
  Community outreach and Advocacy.
  
  Education/Experience Required:
  Education Required: High School diploma or GED Experience Required: Typically requires a minimum of 5 years experience in social services including experience in a supervisory role
  
  Knowledge, Skills & Abilities Required:
  •Good verbal and written communication skills. Ability to work as a team member. •Competent in Windows platform. •Good organizational skills •Ability to handle difficult calls and maintain professional conduct •Willingness to be flexible with job responsibilities •Past supervisory skills

  Physical Requirements and Working Conditions:
  • This position requires travel, therefore, will be exposed to weather and road conditions. • Operates all equipment necessary to perform the job. • Exposed to a normal office environment. • Able to work nights/weekend hours when necessary
