INTRODUCTION:

The Physician serves as a medical provider at the Siletz Community Health Clinic. Using the medical home approach to providing care, providers provide comprehensive care to both tribal members and community members.  



DUTIES:

  • Provides medical screening, evaluation, diagnoses and treatment of heath problems, and routine outpatient surgeries and procedures.
  • Provides emergency medical care.
  • Coordinates referral services with appropriate staff, i.e., Purchase Referred Care Services, Medical Social Worker, if patient’s medical needs are beyond the limits of the facility or practitioner.
  • Participates in and supports the Clinic’s Quality Assessment and Improvement program.  
  • Legibly documents all patient visits on the Patient Care Component form immediately following the encounter. Entries in the patient medical records are to be in the standard SOAP format.
  • Provides positive participation in committees, as assigned.
  • Provides long-term and on-going treatment and education for chronically ill patients and their families.
  • Assists with safety, emergency preparedness, and disaster planning.
  • As necessary, may serve in other areas of incumbent’s discipline.


Requirements

REQUIREMENTS:

Graduate from an accredited School of Medicine, and completion of internship and residency, as a family practitioner or internist.


Ability to:

  • Achieve and maintain appropriate Board certification, and licensure with the State of Oregon.
  • Possess, or be able to obtain, licensure and registration for the prescription and dispensing of narcotics and controlled drugs.
  • Work with a wide variety of people, ability to communicate effectively, and relate to tribal and community members, staff, and federal, state, and local health agencies.


Other:

  • Possess, or be able to obtain, ACLS Certification.
  • Must be eligible for malpractice insurance.
  • May be required to obtain hospital privileges.
  • Must possess a valid driver's license, insurance and be eligible to drive a GSA vehicle.
