M/W and every other Friday at the Troy location, and T/Th and every other Friday at the Warrenton location. Job Summary:

Integrates behavioral health services into clinic settings. Conducts patient interviews, complete assessments and provide brief interventions and referrals to treatment. Counsels patients and family members and refer patients to family and community assistance agencies.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Provides decision support for treatment planning, and assists in the implementation and monitoring of biopsychosocial treatment plans.
  • Counsels patients and family members through individual and family sessions by using brief solution focused therapy.
  • Counsels patients and family members through individual and group sessions to understand, accept, and follow recommendations given by their Primary Care Physician (PCP).
  • Works with primary care team to treat and manage patients with emotional and/or chronic physical health problems.
  • Develops interventions and sets goals for behavioral modifications.
  • Provides on-going psychosocial and behavioral health outreach.
  • Develops collaborative relationships with community organizations in order to provide resources to patients for their various social needs.  
  • Establishes and maintains relationships with psychiatrists to help co-manage patients needing pharmaceutical interventions.  
  • Works in a constant state of alertness and safe manner.

EDUCATION

  • Master's degree in Social Work


EXPERIENCE

  • Two years' experience

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
