Schedule: Full-Time, Monday-Friday, 8am-4:30pm
Highlights of the position:
Be the bridge between patients and the resources they need to thrive.

Serves as an ambassador for PCHC’s primary care clinics, building trust and connection with patients and families.

Screens for social determinants of health (SDoH) and identifies unmet needs through direct engagement.

Partners with providers and care teams to create care plans, coordinate referrals, and connect patients to community resources.

Provides hands-on support with essentials like food, housing, transportation, and financial/legal services.

Participates in crisis intervention, offering compassionate support and exploring alternatives to hospitalization when appropriate.

Offers motivational interviewing, goal-setting, and encouragement to promote wellness and independence.

Serves as a trusted point of contact across clinics, helping patients navigate care and access the services they need.
- Is a good steward of HIPAA. Understands and follows applicable HIPAA laws and regulations and PCHC policies.

Education and Experience:
- High School Diploma (or equivalent) required. Relevant post-secondary education preferred.
- Minimum of two years’ experience in a primary healthcare and/or crisis setting required.
- MHRT-C Certification preferred
- Case Management experience preferred · Previous experience with electronic medical record preferred.
- Obtains MCD Global Community Health Worker (CHW) competency and training as soon as able to enroll into an available class as designated by director and supervisor (preferably within 6 months based on availability).
- Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
- Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required. Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individual’s ability to perform the responsibilities of the position in accordance with PCHC’s Automobile Safety and Background Check Policy.
