OCCUPATIONAL THERAPIST REGISTRY (OT) - HOME HEALTH
LOCATION: FAR WEST SUBURBS
HOURS: DAYS REGISTRY WITH WKD AND HOLIDAY ROTATION
Major Responsibilities:
- Evaluation and implementation of Occupational Therapy treatment programs for patients with multiple diagnoses, safely and effectively.
- 1)Perform patient evaluations and re-evaluations and establish and implement appropriate treatment programs individually or in conjunction with the COTA or student
- 2)Periodic re-evaluation utilizing judgment for progression/modification
- 3)Implements, monitors, progresses, and modifies treatment plan as indicated to attain goals.
- 4)Instruct patients in home exercise program, use of ADL equipment and provide Patient/family education as indicated.
- 5)Schedules patients according to priority/needs.
- 6)Exhibits an average weekly productivity of at least 65% direct patient care for inpatient and 80% for outpatient.
- Documentation of patient evaluation, progress and discharge
- 1)Prepare, maintain and transmit accurate and timely patient treatment records including evaluation, progress and discharge notes.
- 2)Observe and record patient’s progress throughout the treatment period to assure continuity of care and provide accurate records.
- 3)Complete daily charge sheets, productivity, attendance records, charges and services rendered.
- Coordination of activities with other Physical Medicine personnel.
- 1)Shares treatment/office space.
- 2)Supervises COTAs/technicians/aides while performing Occupational therapy treatments as appropriate
- 3)Maintains O.T. equipment in proper working condition and reports any malfunction to the Inpatient/Outpatient Supervisor
- 4)Maximizes use of support personnel by delegating as appropriate
- 5)Follows hospital infection control and safety protocols
- 6)Assists in, taking and delivering accurate messages and other clerical tasks
- 7)Assists with defined responsibilities for the OT department in the absence of the Inpatient/Outpatient supervisor
- Participation in educational / professional development activities
- 1)Keeps informed of changes occurring within Physical Medicine Department and Advocate Healthcare.
- 2)Presents 1-2 inservices per year.
- 3)Attends and participates in departmental meetings on regularly scheduled work days.
- 4)Attends a minimum of two continuing education courses. May be Advocate sponsored or outside sponsored courses..
- 5)Keeps abreast of current practices via literature, networking, internal/external resources, etc.
- 6)Participates in PI programs and on PI teams as needed.
- 7)Completes mandatory competencies in a timely manner.
- 8)Assists in planning/presenting at health fairs, career days and other community programs as needed.
- 9)Provides inservice training to hospital personnel regarding therapy techniques and equipment as needed.
- Promotion of Occupational Therapy
- 1)Assists in planning/presenting specialty seminars.
- 2)Participates in health fairs, career days and other community programs as needed.
- 3)Assists with marketing programs, both planning and implementation
- 4)Supervise and mentor student volunteers.
- Provision of clinical education programs
- 1)Supervise and evaluate students on learning progress and cooperate with the school in an effort to improve and update the clinical experience for students
