Your Specific Responsibilities:
This position provides professional nursing services in the care and treatment of patients within DOH Duval community health care centers and satellite service provision sites. The candidate who will fill this position will work under the supervision of the Assistant Community Health Nursing Director, Registered Nursing Consultant, and/or in conjunction with the medical provider and nurse leadership team. A current Florida Nursing License is required.

- Responsible for processing, coordinating, and following up on referrals with other agencies and documenting referrals in the electronic health record within 48 to 72 hours of initiation of referral.
- Pulls medical records for laboratory results and provider's review. Files medical records. Ensures positive communication when answering telephones. and assists with telephone messages as appropriate.
- Provides exemplary customer service during telephone and walk-in triage as necessary in consultation with the provider or registered nurse.
- Correctly makes assessments of and documents patient's health status.
- Accurately applies nursing principles and techniques in the direct care and treatment of patients.
- Under the direction of the medical provider, evaluates, treats, and provides health education and follow up with appropriate cultural competence.
- Positively assists provider in clinic session.
- Interprets and implements provider's orders and recommendations to patients appropriately.
- Administers vaccines to patients, documents vaccine administration in FLSHOTS, and the medical record. Maintains vaccine inventory and provides Vaccine Information Statements (VIS) to patients and/or parents.
- Vaccine accountability: prints Physical Inventory Report (PIR) and counts refrigerated vaccines with a nurse at the end of the day.
