Career Team is looking to hire a Retention Case Manager who is experienced in workforce development to serve clients of the Pathways for Access to Careers and Employment Workforce Development Program (PACE) Team. The approach of the PACE Program emphasizes that there is no one-size fits-all solution for clients needing education and or training support. The Retention Case Manager will provide case management and follow-up services to employed clients and obtain proof of employment while monitoring benchmarks and provide barrier remediation services as needed. This position is expected to be 100% onsite.

Your Impact on Career TEAM’s Success:

  • Manage a caseload of PACE clients once they are placed into employment and provide services if needed to help them maintain employment
  • Develop and manage a comprehensive working relationship with all stakeholders, including clients, governmental agencies, employment partners, service providers, etc.
  • Obtain paystubs, offer letters, or letters of promotion from employed clients to verify proof of employment and document in accordance with program requirements
  • Identify and assist with mitigating barriers to program participation and sustainable employment after program, including but not necessarily limited to, health, mental health, housing, childcare, domestic violence issues
  • Maintain an updated client database and track client engagement in services, and accurately document case management activities in the State database daily
  • Monitor and supervise the client’s employment placement and benchmarks to track progress and encourage promotion within the job 
  • Prepare reports of activities and achievements
  • Collaborate with Case Managers and Leadership to develop strategies for successful client engagement after employment placement 
  • Advocate for clients' needs and rights, providing advocacy and support as necessary
  • Maintain accurate and up-to-date records of client interactions and outcomes
  • Ensure compliance with data privacy and confidentiality policies and procedures
  • Approach each day and task with a “ZAG” mindset
  • Other duties and projects as needed for the success of the program

The Ideal Qualifications for this Position Include:

  • Associate’s degree from an accredited college or university, OR a Minimum of two (2) years of experience in case management, human services or a position that demonstrates the ability to perform the duties as described
  • Effective and engaging communicator with strong interpersonal skills
  • Analytical and organizational skills to manage outreach efforts efficiently
  • Resilient and outcome-driven with a focus on client engagement and program success
  • Knowledge of Government Workforce regulations such as PACE, EARN, TANF, SNAP, etc.
  • Computer-literate with knowledge of relevant software and tools.
  • Ability to work in a dynamic team environment - must have excellent teamwork, communication, and interpersonal skills
  • Detail-oriented with strong organizational, analytical, research, and problem-solving skills
  • Knowledgeable about the local region and relevant partners
  • Multilingual or Bilingual preferred 
  •  A Valid Driver’s License

Salary: $55,000/yr

Employment Type: Full-Time, Non-exempt Employee
