About the role

As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient.  The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members.  Collaborates with the patient's Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Required Qualification:

  • Washington Registered Nurse License upon hire.

Preferred Qualifications:

  • Bachelor's Degree in Nursing.
  • 3 years current experience in a health care environment with home health case management experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
