This position works on a small team dedicated to implementing innovative models of care for HIV and syphilis testing in the emergency department and providing linkage to treatment and comprehensive prevention services. The coordinator role will be an emergency department facing role, and will be essential for the initiation and success of a new universal opt-out HIV and syphilis triage testing process for emergency department patients. This role will include nursing and provider education as well as implementation of universal HIV and syphilis screening in emergency department patients. Comprehensive navigation services include a range of activities to ensure unfettered access to HIV and syphilis treatment and prevention services.

Job Description

Essential Functions

  • Responsible for providing education to nursing staff, ED techs, and ED providers about universal opt-out HIV and Syphilis screening, current CDC recommendations for screening, and data behind this initiative
  • Identify and educate nursing champions in the department to facilitate widespread acceptance and utilization of this initiatve
  • Identifying and arranging treatment for new diagnoses of syphilis
  • Collaborating with the Philadelphia Department of Health to manage syphilis diagnoses and contact tracing/partner support
  • Assist with linking HIV positive patients into HIV medical care by performing a range of supportive, logistical, and navigation duties to ensure prompt care initiation;
  • Systematically increase HIV treatment and prevention referrals by utilizing the electronic medical record (EMR) to identify appropriate patients and engaging them in healthcare services. Return inquiries of HIV test results and engage patients in prevention counseling.
  • Enhance positive healthcare outcomes and community awareness about syphilis and HIV/AIDS, impacting community health through education, community outreach and engagement, linkage, referrals and direct service delivery.
  • Track progress using Careware, EMR, and MS excel as appropriate; use data to identify needs assessment.
  • Contribute to program reporting.
  • Collaborate with community partners.
  • Disseminate program results and best practices.
  • Comply with reporting requirements and grant deliverables
  • Participate in discussions in staff meetings and case consultations;
  • Coordinate efforts to recruit and retain patients in HIV prevention services.
  • Promptly attend scheduled staff meetings, in-services and trainings.
  • Conduct HIV prevention and sexual health risk reduction education and counseling. Engage HIV negative patients in psychoeducation on HIV/AIDS disease acquisition and prevention services, including testing, behavioral and biomedical HIV prevention interventions.
  • Link patients to ancillary services: free condom access, mental/behavioral health supports, and other community services.
  • Respond to HIV+ tests within 2 working hours and HIV- tests within 8 working hours

Minimum Education and Experience Requirements:

Education:

Bachelors degree required

AND

Experience:

3 years bedside medical experience (nursing, ED medical provider, medical school, PA school)
