Your Mission:Reporting to the Behavioral Health Program Coordinator, the Behavioral Health Case Manager works collaboratively with all CASL program departments to assess and address the behavioral health needs of entire households. This role involves visiting clients of all ages, conducting interviews, documenting assessments, and developing service plans to help clients overcome challenges and achieve their goals.

Your Impact:As a Behavioral Health Case Manager you will:
Key Responsibilities:

Client Advocacy and Assessment (70%):

  • Facilitate regular interdepartmental team meetings to implement the Client Advocacy Unit Model across the agency.
  • Develop and maintain relationships with community partners to connect clients with additional services as needed.
  • Conduct behavioral health intakes and reassessments to understand clients' needs and social determinants of health.
  • Evaluate treatment options and create comprehensive service plans, collaborating with clinical or medical professionals as required.
  • Provide case management, crisis intervention, community support, and counseling/therapy services under close supervision.
  • Hold quarterly stakeholder meetings to gather feedback from clients and community stakeholders.
  • Coordinate service delivery with primary care providers, care coordination entities, and other external entities.
  • Ensure referrals for substance-related and addictive disorder treatment services are provided as needed.
  • Conduct community outreach and education to increase awareness of behavioral health issues.

Administrative Responsibilities (30%):

  • Maintain client records and ensure timely clinical documentation of all sessions.
  • Operate in compliance with all applicable laws, regulations, and CASL policies.
  • Collaborate with the Behavioral Health Program and Marketing and Community Department on mental health media campaigns and website updates.
  • Conduct quarterly quality assurance reviews of client records and clinical care.
  • Ensure compliance with grant requirements and internal policies.
  • Participate in professional development opportunities to maintain mental health-related licensure/certification.
  • Manage data using Salesforce and Electronic Health Record Systems.

Must Haves - To thrive in this role, you should have:

  • Bachelor’s degree in Psychology, Social Work, or a related field.
  • Bilingual proficiency in spoken and written English and Chinese (Cantonese and/or Mandarin)
  • Mental Health-related license/certificate to practice in Illinois or eligibility to obtain one, (e.g., LMFT, LCPC, LCSW, LSW, LPC)
  • Proficiency in spoken and written English 
  • Excellent communication skills and conflict resolution abilities.
  • Proficiency in Microsoft Office and G-suite, or willingness and ability to learn
  • Ability to maintain timely clinical documentation and client electronic health records.
