Job Summary: 

Charter Oak Health Care is seeking a dedicated and compassionate Community Health Worker to join our team. The ideal candidate will work closely with community members to improve health outcomes by providing education, support, and resources. This role involves outreach, advocacy, and collaboration with healthcare providers to address the social determinants of health.

Key Responsibilities:

• Conduct community outreach to identify individuals and families in need of health services.
• Provide health education and information to community members on various health topics.
• Assist clients in navigating the healthcare system, including scheduling appointments and accessing services.
• Advocate for the health needs of the community and connect clients with appropriate resources.
• Work with healthcare providers, social workers, and community organizations to ensure comprehensive care for clients.
• Maintain accurate records of client interactions and services provided.

Requirements

Qualifications:

• High school diploma or equivalent; additional training or certification in community health is preferred.
• Previous experience in a community health or social services role is preferred.
• Strong communication and interpersonal skills; ability to work independently and as part of a team; proficiency in using electronic health records (EHR) systems.
• Understanding of the social determinants of health and the ability to address barriers to healthcare access.
• A genuine passion for helping others and improving community health.
• Ability to work effectively with diverse populations and understand cultural differences.
• Willingness to adapt to the dynamic needs of the community and the healthcare environment.
• Commitment to maintaining the highest standards of confidentiality and ethical conduct.
• Bilingual Spanish/English preferred
