What You’ll Do:
• Through community outreach, provide ongoing support to consumers to help them function independently and access mental health services.
• Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
• Assess the need for crisis intervention and assist in psychiatric emergency situations.
• Encourage wellness and recovery while maintaining individual dignity and respect.
• Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.
Job Requirements:
• Bachelor’s Degree in Social Work, Psychology or related field from an accredited college or university.
• At least 2 years of experience with community-based mental health services is preferred.
• Intermediate computer skills including Microsoft Word & Excel.
• Ability to complete documentation in an Electronic Health Record (EHR) system.
• Valid driver’s license and acceptable driving record.
• Excellent communication, planning, and organizational skills.
• A compassionate, flexible, and client-focused approach to care.
