Position: After Hours and On-Call Registered Nurse

Location: Benton Franklin County

Type: Full Time, Exempt 


Anticipated Schedule: On call 7 shifts per 2 week pay period. Shifts are from 4:30 pm to 8 am. On call from home triaging calls and making home visits. Occasional support at the hospice house during scheduled shifts. One additional "back up" shift per month.

POSITION OVERVIEW

As a Hospice After Hours and On-Call Registered Nurse (AHOC RN), you will play a vital role in providing compassionate and specialized care to patients and families during non-business hours in a hospice setting. You will be responsible for assessing patient needs, coordinating care, providing emotional support, and ensuring the highest level of comfort for patients in their final stages of life. This role requires excellent clinical skills, empathy, flexibility, and the ability to work independently


MINIMUM POSITION QUALIFICATIONS

1. Current Washington State license as a professional registered nurse.

2. At least two years of post-education nursing experience, preferably with one year in a hospice, palliative care, or oncology setting.

3. Ability to work independently, make sound decisions, and remain calm under pressure in urgent situations.

4. Must be available as scheduled outside of our regular business hours, for evenings, weekends, and holidays, as part of the on-call nursing staff.

5. Excellent communication and interpersonal skills to interact compassionately with patients, families, and the interdisciplinary team.

6. Proficiency in using electronic medical records (EMR) systems and Intermediate computer skills.

7. This position is designated as Safety Sensitive. Must be able to pass a pre-employment drug screen which includes cannabis testing in Washington State.


CLINICAL STAFF POSITION QUALIFICATIONS

1. As required per position/credentialing; successful completion of Blood Bourne Pathogens Training.

2. Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS)

3. (Hospice House) Washington State Food Worker Card issued by the Department of Health

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

Operational Responsibilities:

1. Operate within the framework of policies and procedures, state and federal regulations, and the patient’s plan of care.

2. Complete all documentation within agency guidelines and requirements after on-call services are provided to allow for appropriate care and shift-change transition.

3. Maintain regular communication with nursing staff and leadership regarding problems or patients from on-call hours.

4. Document and communicate all calls, interventions, care plan changes, and physician contacts in a manner consistent with policy and procedure.

5. Document patient care reflecting nursing interventions, patient response to care, and focusing on patient goals.

6. Attend and participate in required meetings and training.

7. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.

8. Maintain regular communication with the Hospice Medical Director or delegate concerning patient and family care needs and response to interventions.

9. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.

10. Inform the Hospice Manager Support Clinical Supervisor or Administrator On-Call to review patient needs or inform them of unusual or potentially problematic patient/family issues.

11. Must provide support and coverage at the Hospice House as assigned by the administrator on call.

Patient Care Responsibilities:

12. Must be available by phone and in person to meet the needs of the patients and to provide management and care of patients.

13. Attend patients’ deaths to provide support to the patient’s family and notify the Inter-Disciplinary Team.

14. Respond to medical or emotional crises and/or death of patients and make home visits to provide direct care or bereavement resources as appropriate.
