CASE COORDINATOR

Job Category: Behavioral Interventionist (BI) Program

Job Status: Full-time

Reports to: BI Program Manager


Job Summary: The Case Coordinator provides supervision to frontline BI staff and case management to families served in the BI Program.


Responsibilities

Case Management

  • Family satisfaction: ensure families are given proper support, advocacy, resources, communication
  • Complete required assessments with families, adapting to the needs of the client and family
  • Attend court appointments + other professional family-based meetings
  • Matching BIs to homes, resolving issues, attend team meetings
  • Review Incident Reports, follow-up with family, if needed
  • Facilitate Referral Triage, Intake & Meet & Greets for each family
  • Submit approved Recommendation & Approval Letters for services
  • Create Behavioral Plan for each client
  • Provide updated communication during the Admissions & Hiring processes
  • Complete Quarterly Meetings & Bi-annual Assessments with each family
  • Write/submit renewals
  • Update SAM, electronic health record, & profiles
  • End of Month Reports
  • Case Closure & Aftercare Follow-up meetings,
    assessments, and documentation
  • BI Quarterly Progress Report


Administrative Leadership & Supervision

  • Employee satisfaction, build a strong sense of team
  • Ensure BI staff are given proper support/advocacy
  • Conduct interviews & participate in the on-boarding process of BIs
  • Assist in BI monthly meetings
  • Help conduct client/staff matchings
  • Conduct supervisory evaluation of BIs, communicating  concerns with Program Manager
  • Provide regular in-home observation shifts of BIs
  • Track missed shifts & check shift notes
  • Complete necessary corrective actions & terminations
  • Update/track employee timecards in the payroll system
  • Provide support to BIs during OHI Investigations
  • Build relationship with other partner agencies & community stakeholders
  • Maintain strong, open communication with BI Team
  • Maintain CPR/First Aid certification
  • Collaborate with branch offices for program fidelity
  • Attend required
    trainings for professional development


Outcome Planning/PQI  

  • Complete agency reports & statistics
  • Track & report program outcomes & QA
  • Maintain resource provider files compliance
  • Complete Critical Incident Reports with Program Manager
  • Complete Incident Report Reviews


Qualifications & Preferred Skills  

  • MA/MS (or BA/BS +2 years working in non-profit social service agency) in social work, psychology, public administration, or related human services field
  • Previous experience working with at-risk populations
  • Familiarity with the child welfare system
  • Resourcefulness in setting priorities, proposing new ways of creating efficiencies, guiding investment in people & systems
  • Understanding of program development, implementation & evaluation
  • Organizational & planning skills
  • Commitment to principles of support & advocacy for foster & adoptive families
  • Strong verbal & written communication skills   
  • Leadership, team management, interperso
