Job Summary

 

The   Quality Assurance Specialist/Certified Investigator (QAS/ CI) will   support the work of the Director of   Quality Assurance and Regional Directors with the primary role of Certified Investigator. The position requires travel to   various site locations in the surrounding counties of Philadelphia. Once assigned   to an investigation, often with little to no   advance notice, the QA Specialist will be responsible for initiating the   investigation within the required timeframe as dictated by program   regulations and agency policy, completing all steps of the investigation   including interviewing involved individuals receiving services and staff;   collecting, documenting, and preserving evidence in line with regulatory   requirements; writing the investigation report; submitting the report to the   Incident Manager for review/approval; and final submission of the report   based on agency policy. The Quality Assurance Specialist/Certified Investigator will   support the work of quality reviews, training development and delivery, and   other tasks and duties as assigned.

Requirements

Essential Functions

 

· Conducts   investigations according to regulations, policy and procedures.

· Ensures   state notification of suspected abuse, neglect, exploitation, rights   violations serious injuries, and other forms of mistreatment in coordination   with program management. 

· Begins   investigations within required timeframe, including being on-site within 24   hours of incident discovery. 

· Develops   investigation questions.

· Interviews   all involved parties, including alleged victim(s), witness (es), and alleged   target(s).

· Preserves, collects, examines, organizes, documents, and stores evidence. 

· Writes   investigation reports in a clear, concise, and coherent manner, with focus on   facts, such that any reader would be able to understand the chain of events.

· Submits   required information to state incident reporting site in accordance with   program requirements and regulations.

· Participates   in Certified Investigator Peer Review process and other related agency   processes as needed. 

· Utilizes   critical thinking, objectivity, confidentiality, flexibility, person-centeredness,  and thoroughness throughout.

· Assists   with internal program auditing to include evaluation of participant records,  medication, and physical site compliance in accord with regulations and   agency policy. 

· Assists   with the collection and entry of data applicable to identified quality   objectives. 

· Provides   support as needed with program licensing activities. 

· Participates   in special projects or quality initiatives as needed. 

· Assists   with generating and drafting reports as directed. 

· Functions   as a point of contact in the absence of the Directory of Quality   Assurance. 

· Assists   in the development of training curriculum as assigned to meet regulatory and   funding requirements and to improve service delivery and overall program   quality.

· Participates   in the development and training of employees to improve their work quality   and job knowledge.

· Attends   other departments meetings as requested to share information on systems   changes.

· Serves   as a resource and support to staff, supervisors and directors.


Other Functions

· Provides   managerial support to programs as assigned during periods of vacancy. This   includes temporary assignment to various programs across the organization on   an as needed basis. 

· Attends   and participates in regular supervision.

· Provides   on-call coverage as needed and assigned.

· Performs   job within framework of agency, site, and professional standards.

· Participates   in staff meetings and training programs.

· Adheres   to agency/site policies and procedures including HIPAA requirements.

· Projects   a positive attitude and image to staff, participants, family members, and   outside contacts.

· Uses   appropriate communication channels.

· Completes   assignments independently and within specified time frames.

· Participates   in achievement of agency objectives, goals, and mission.

· Follows   agency dress code.

· Adheres   to supervisor's requests and displays ability to take direction.

· Shows   initiative and demonstrates good judgement.

· Follows   instructions accurately and thoroughly.

· Works   as scheduled to assure coverage.

· Performs   job in a safe manner.
