The Mechanical Circulatory Support Nurse Coordinator (VAD Coordinator) has responsibilities for assessment, coordination of care and education of inpatients and outpatients being evaluated for VAD placement or who are on VAD therapy. The VAD Coordinator works closely with the Advanced Heart Failure and Transplant Team in coordinating patient care, assisting in the operation of MCS clinic hours, and participating in the 24/7 on-call schedule.

Job Description

  • Completes clinical evaluation of potential candidates for VAD intervention including ordering and   reviewing tests, labs and other data.
  • Review results with referring heart failure physician and manage any additional follow up required as part of evaluation.
  • Compile information related to evaluation and present at multidisciplinary weekly team meetings.
  • Provide ongoing education at every phase of the MCS continuum to patient and their caregivers
  • Coordinates intra-operative device assembly and management during device implant as needed.
  • Coordinates long-term management and follow-up care for outpatients post MCS implant.
  • Collaborates with Advanced Heart Failure Providers to implement clinical interventions for all MCS patients throughout their care continuum.
  • Participates in the development and maintenance of current education materials and provides ongoing education sessions to maintain staff competencies.
  • Participate in outpatient clinic hours, inpatient rounding and the 24-hr/7 days on-call schedule rotation
  • Triage pre and post-VAD outpatient phone calls and communications during regular and off-hours when on call in collaboration with the physician care team.
  • Maintain program policies and protocols for patients in all phases of the transplant process.
  • Maintain regulatory requirements for Joint Commission for MCS program.
  • Collaborates effectively with all multidisciplinary team members and all care teams/disciplines involved with care of the VAD recipient
  • Participates in the development and implementation of continuous quality improvement programs.
  • Participates in multidisciplinary transplant meetings, including, but not limited to patient selection committee, rounds and huddles.
  • Provides support and education to surrounding staff and other health professionals regarding the device, its implications for the patient and how to troubleshoot alarms and alerts. Assists with trouble shooting onsite, as needed.
  • Verifies patient knowledge base regarding the equipment used for mechanical support prior to discharge from hospital.
  • Performs other related duties as identified.
