Primary Responsibilities:
- Oversee the continuum of care for Re-entry Community Safety Program (RCSP) clients, including pre-release engagement, Multi-System Care Planning Team (MSCPT) coordination, and post-release intensive services.
- Serve as the primary care coordinator for clients in Behavioral Health Court (BHC), ensuring clinically appropriate services are delivered based on individual needs.
- Develop, implement, and evaluate personalized treatment goals and plans in collaboration with a multidisciplinary treatment team.
- Participate in weekly Behavioral Health Court hearings and meetings to support client progress.
- Educate and support clients' families while advocating for clients’ rights and preferences. Consult with community agencies, including DSHS, housing authorities, and court systems, to facilitate care coordination.
- Provide crisis intervention and collaborate with Designated Crisis Responders when necessary.
- Monitor clients for Less Restrictive Alternative (LRA) compliance, facilitate extensions, and assist in determining when revocations are appropriate.
- Engage with clients post-release to encourage continuation of care and services as needed beyond the RCSP program’s maximum benefit period.
