JOB SUMMARY:

The Home Infusion Coordinator at Polaris Pharmacy Services is responsible for coordinating and managing the delivery of home infusion therapy services to patients. This role ensures that patients receive high-quality, timely, and efficient care while maintaining compliance with all regulatory and company standards. The coordinator acts as a liaison between patients, healthcare providers, and the pharmacy to facilitate seamless service delivery.

DUTIES/RESPONSIBILITIES:

  • Coordinate the initiation and continuation of home infusion therapy services for patients.
  • Serve as the primary point of contact for patients, healthcare providers, and internal teams regarding home infusion services.
  • Ensure accurate and timely communication of patient information and therapy requirements to all relevant parties.
  • Schedule and oversee the delivery of medications and supplies to patients' homes, ensuring timely and accurate fulfillment.
  • Monitor patient therapy progress and address any issues or concerns in collaboration with healthcare providers.
  • Maintain comprehensive and accurate patient records, ensuring compliance with all regulatory and company standards.
  • Collaborate with pharmacists, nurses, and other healthcare professionals to optimize patient care and outcomes.
  • Provide education and support to patients and their families regarding home infusion therapy and related processes.
  • Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.
  • Stay informed of industry trends and best practices to continuously improve the home infusion program.

REQUIRED SKILLS/ABILITIES:

  • Strong knowledge of home infusion therapies, including medication administration and patient monitoring.
  • Excellent organizational skills with the ability to manage multiple tasks and prioritize effectively.
  • Proficient in using healthcare management software and electronic medical records.
  • Exceptional communication skills, both verbal and written, to interact with patients, families, and healthcare professionals.
  • Ability to work independently as well as collaboratively within a team environment.
  • Strong problem-solving skills and the ability to make informed decisions under pressure.
  • Demonstrated commitment to providing high-quality patient care and maintaining patient confidentiality.

EDUCATION and/or EXPERIENCE:

  • High School diploma or equivalent required
  • Minimum of 3 years of experience in home infusion therapy or a related healthcare setting.
  • Proven experience in coordinating patient care and managing infusion services.
  • Experience working with interdisciplinary teams to deliver comprehensive patient care.
