Position Overview

Salary - $46,000

Hours - flexible and non-traditional hours required.

 

Summary

Under the general supervision of the Director of Case Management or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family of origin, foster parents, and community partners, providing advocacy, professional services, and support toward the ultimate goal of living safely and successfully in the community. 

 

Responsibilities

  • Actively supports, represents, and extends the mission, vision, and values of the organization. 
  • Completes or revises the Family Risk Assessment with all family members that live in the home to assess the family’s strengths and needs, as well as risk of harm to the child/children in the home, monitoring safety, and immediately reporting incidents involving harm or risk of harm to children. 
  • Maintains contact with families through regular and planned, as well as unannounced, visitation of parents and children. 
  • Provides case management, transportation, parent education, counseling, community resource linkage, advocacy, and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served. 
  • Prepares for and participates in administrative and court reviews of the case plan and other court proceedings. 
  • Works with Foster Care Programs to coordinate visits with the Treatment Coordinator in the Foster Family’s home to improve communication. 
  • Pursues permanent custody and planning for adoption or other permanent substitute care for children who cannot be returned to their birth families. 
  • Performs duties on-call as outlined in on-call description when called upon to respond to after hour emergency situations. 
  • Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements. 
  • Completes all required documentation and training in a timely manner. 
  • Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals. 
  • Utilize Children Welfare Best Practice Principles in decision making. 
  • Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis. 

 

Minimum Qualifications

  • Bachelor’s Degree in Social Work or comparable Human Services field from an accredited institution. 
  • 2 years of work experience working in direct service with youth and families strongly preferred. 
  • Must have a valid driver’s license, reliable transportation, automotive insurance requirement of 100,00
