The Community Engagement Team lead role is to improve health outcomes for persons within identified communities through linking identified community need with available resources. Navigators partner with Community Services leadership to facilitate integrated service delivery for residents of assigned communities Works as part of the Community Engagement team in ongoing program improvements and tracking project outcomes while training and coaching team members in the areas of job responsibilities and policy and procedures.

Requirements

• Strong knowledge of data analysis, excel, and dashboards.
• Knowledge usually acquired through Associates Degree in social services or related field.  Bachelor’s Degree preferred.
• Professional demeanor and excellent communication skills, written and verbal, to relate to people of varying socioeconomic class with differing lifestyles or cultural values and to interact with community based service organization professionals.
• 3 years of experience working with families of diverse ethnic, cultural and socioeconomic backgrounds.
• Strong time management and organizational skills to maintain project tracking activities and manage multiple priorities. Some supervisory experience preferred.
• Strong analytical ability to assess various crisis situations and recommend appropriate plan of care.
• Ability to work independently with limited supervision.
• Requires a valid driver's license, acceptable driving record, minimum liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.
• Requires personal computer and word processing knowledge in order to enter and retrieve related information for effective reporting and correspondence.
• Demonstrated good standing within the community and an interest in improving outcomes for the neighborhood(s).
• Demonstrated skills in creative problem-solving, community engagement, outreach and has in-depth knowledge of the assets and challenges of the community to be served.
• Proven ability to work with medical professionals to ensure follow up and coordination of recommended medical management.
