Position Summary
As a member of the Rehabilitation staff, plans and implements specific programs for individual patients and patient groups according to the principles and practices of occupational therapy.
Primary Responsibilities
• Screens all new admissions and any patients referred by the medical team through gross assessment and makes recommendations to the nursing staff
• Evaluate assigned patients, gathering data from a variety of sources, such a patient records, interviews, observation and team members
• Develop treatment plans for assigned patients including specific goals and activities. Reevaluate and modify plan at periodic intervals and as necessary.
• Review requisitions for physical therapy. Interview patients, evaluate patient's condition and needs.
• Determines proper equipment usage, application, adjusting as necessary, in administration of such treatments as exercise including gait training, hot packs, ultrasound and electrical stimulation.
• Prepares written documentation as required by the profession and Sherrill House, such as evaluation results, treatment plans, progress notes and other reports.
• Communicates observations, facts and comments with nursing and interdisciplinary team members, individually and through team meeting presentations to ensure efficient coordination of the overall care plan
• Provides guidance and direction to those assigned to assist in physical therapy services and serves as clinical education coordinator in the supervising of student programs
• Maintains department policies, procedure, objectives and quality assurance program
• Attends meetings and participates on committees as required
• Enhances professional growth and development thorough participation in educational programs, current literature, in-service meeting, workshop, etc.
• Provide in-service training specific regarding physical therapy to other rehab staff, nursing staff, patients, and family members.
• Performs other duties as directed by supervisor.
• Maintains accurate billing information in accordance with Sherrill House procedures on a daily basis.
• Maintains confidentiality of all patient, facility and physician related information.
• Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
• Maintain an excellent working relationship with the medical profession and other health related facilities and organizations.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.
• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly reviews.
Qualifications
License: Must have current state license
Experience: A minimum of two years of experience in a therapist capacity in a hospital, skilled nursing care facility, or other related medical facility and training in rehabilitative and restorative therapy practices.
Educational Requirements: Graduation from an approved school of physical therapy with bachelor's degree in physical therapy (new grads welcome!)
