Salary: $60,000 - $65,000
SUMMARY
The Medical Case Manager provides support and case management services to an assigned caseload of homeless individuals, including, but not limited to, those affected by HIV/AIDS, living at Crossroads. The goal is to assess clients and offer a continuum of AIDS-related supportive services that will enable HIV positive homeless people to remain connected to primary health care and transition into permanent or transitional housing with an appropriate income stream. The person in this position provides tools for basic living, via basic life skills, and referrals to training services, as appropriate.
Medical Case Management Service Delivery
- Maintain confidentiality in all matters pertaining to residents.
- Provide general orientation to new residents including the review of program guidelines and penalties for non-compliance with designated shelter guidelines.
- Set up and maintain well-organized HIV case management files that include assessments, service plans, progress notes and other supporting documents that are in compliance with Crossroads' contracts standards and requirements. (Use HMIS to input: Intake, exits, Input services, access and utilize assessment tools, progress notes, report building for record keeping and reporting purposes).
- Provide case management and advocacy services to 80 residents on-site at Crossroads.
- Meet weekly with homeless individuals to assess/prioritize individual goals.
- Provide information and referrals to increase residents' ability to achieve goals.
- Monitor the progress and activities of residents and adjust goals as needed.
- Conduct weekly group orientation to new shelter residents.
- Assist residents to access a continuum of support services that will enable him or her to: develop income eligibilities as applicable (track income at entry/exit) (track health insurance, e.g., Medical at (entry/exit).
- Provide budgeting skills and set money management guidelines.
- Make connections to medical, mental health, substance abuse resources as applicable
- Provide permanent housing or transitional housing opportunity resources.
General Operations
- Attend and participate in all required staff meetings, weekly supervision meetings, trainings and in-service trainings.

- Must be knowledgeable of Crossroads program guidelines to ensure consistency of approach in holding clients accountable for case management, and program-related activities.
- Writes semiannual progress reports and maintain statistics on Ryan White
- Maintain tracking log data systems for Ryan White and HOPWA contract reporting
- Complete HOPWA quarterly statistical report.

- Track and submit PATH data on spreadsheet monthly
- Attends team decision making meetings on a weekly basis to discuss client cases and devise strategies that support clients' progress and growth.
- Produces a comprehensive individual service plan addressing related needs for every resident on caseload with realistic goals and objectives and an appropriate timeline.
- Coordinates information and services with the referring agency and other appropriate sources.
- Writes incident reports, warnings and write-ups for program violations.
