Responsibilities: As an Admissions Registered Nurse at Snowline Hospice, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. Your duties will include:

• To coordinate and provide care to our hospice patients
• To calm, soothe and empathize with the strong emotions our patients and families are experiencing
• To participate with a team of healthcare professionals in superior level of care for each patient
• To address personal care needs and promote patients’ comfort needs 
• Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
• Assist with observing the patient along with documenting care
• The Admissions Registered Nurse (RN) makes patient referral visits and facilitates patient admission to hospice. 
• Develop a hospice presence in hospitals, SNFs, RCFEs with possibility of attending rounds and care conferences, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff orientation
• Act as a liaison and advocate for Hospice patients admitted to the hospital, SNFs, and RCFEs 
• Assess each consult to determine the appropriated for Hospice admission, based on individual Hospice criteria
