About the role
Concord Post Acute is seeking a skilled Occupational Therapist to join our team. We have multiple openings available for full-time, part-time, and per diem schedules.Key Responsibilities
- Develop and execute personalized treatment plans based on physician orders.
- Provide direct patient care to improve daily living skills and independence.
- Document patient progress, evaluations, and certifications in accordance with all regulatory requirements.
- Collaborate with the interdisciplinary team on patient care, progress, and discharge planning.
- Supervise Occupational Therapy Assistants and train Restorative Aides.
- Assess and facilitate ordering of necessary durable medical equipment.
