We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team and help us achieve our mission of reducing hospitalizations and improving the quality of life for our patients through advanced CHF management and Goal Directed Medical Therapy (GDMT).</p> <p><strong>Job Responsibilities:</strong></p> <ul> <li><strong>Patient Assessment:</strong> Conduct comprehensive assessments to identify early signs of CHF exacerbation, enabling timely interventions to prevent hospitalizations.</li> <li><strong>Treatment Planning:</strong> Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.</li> <li><strong>Medication Management:</strong> Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.</li> <li><strong>Patient Education:</strong> Educate patients and their families on CHF management, emphasizing the importance of medication adherence, lifestyle changes, and recognizing early symptoms of worsening heart failure.</li> <li><strong>Symptom Management:</strong> Proactively manage symptoms through home-based interventions, such as diuretics adjustments and dietary modifications, to prevent acute exacerbations.</li> <li><strong>Coordination of Care:</strong> Collaborate with a multidisciplinary team, including cardiologists, home health nurses, and social workers, to provide comprehensive, coordinated care in the home setting.</li> <li><strong>Follow-Up Care:</strong> Conduct regular home visits or telehealth consultations to monitor patient progress, adjust treatment plans, and provide ongoing support.</li> <li><strong>Emergency Ca
