Job Description

The Associate Director, Privia Care Partners works with the Associate Vice President, Privia Care Partners and other market-based Population Health leadership to ensure the success of our Privia Care Partners value-based and ACO program in the Georgia market. They will be responsible for a wide-variety of areas, including managing value-based contracts and relationships, supporting new practice implementations, physician engagement, new population health initiatives, and operational oversight as the population health team grows.

 

Primary Job Duties: 

 

  • Key stakeholder: the primary point of accountability for overseeing programs and delivering success in Georgia’s Privia Care Partners value-based contracts
  • Establish strong relationships with Privia Care Partners key clients, physicians, and office staff to ensure success in value-based programs and improve engagement.
  • Analyzes population health and utilization metrics to determine how best to deploy clinical staff in partnership with Physician leadership and with support from medical economics teams
  • Trains specific physician practices on how to transform their group and staff to the new world of value-based care, on topics including Quality, Risk Adjustment, and Total Cost of Care
  • Leads regular physician meetings, both group and one-on-one, to review population health training, initiatives, and data.  Also leads ACO Compliance and Board meetings.
  • Collaborate with the market-based population health team to develop and execute strategies to ensure quality benchmarks in all value-based programs are met
  • Partner with physician leaders to drive physician engagement in value-based programs
  • Actively lead and support physician meetings to review population health training, initiatives, and data
  • Provide training and support to providers and care centers on key population health initiatives and workflows to support success in value-based contracts.
  • Meet with key payer stakeholders and contracts in the market
  • Willing to complete chart chase, attestations and other tasks integral to success as necessary
  • Develop staffing structure and onboarding program for all supporting staff
  • Manages staff and communication with staff- creates a positive workplace culture that promotes results through collaboration with physicians, physician extenders, and supporting care center team
  • Perform other duties assigned

The person in this position will be an:

Qualifications

  • 7+ years of experience working in health plans, health systems, and/or provider organizations, with strong operational experience with value/risk-based contracting/performance experience
  • Prior experience in managing staff, preferably large teams
  • Ability to interact with all levels including senior management and influence decision-making
  • Demonstrated ability to work with, train, and engage physicians and office staff
  • Able to grasp both clinical and business concepts
  • Analytical, quantitative, and metrics driven; must know how to interpret value-based program requirements and understand the impact on financial results
  • Ability to manage multiple projects and priorities at one time
  • Must comply with HIPAA rules and regulations

Colorado, New York, New Jersey, California and Washington Residents Only: The salary range for this role is $109,740 to $118,000 in base pay and exclusive of any bonuses or benefits. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
