EHR Chart Correction Specialist

Job Description

The EHR Chart Correction Specialist is responsible for reviewing, analyzing, and correcting electronic health record (EHR) data to ensure documentation accuracy, data integrity, and compliance with healthcare standards and regulations. This role plays a critical part in maintaining the quality and reliability of patient information across the health system.

Essential Functions

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Review and process chart correction requests submitted by clinical and administrative staff; process patient requests for amendment
  • Identify, analyze, and correct documentation errors in the EHR (e.g., misfiled documentation, duplicate records, incorrect patient demographics).
  • Prioritize and assist in resolving complex critical errors (patient safety, patient care, reimbursement, etc.) to mitigate risk and provide resolution.
  • Collaborate with physicians, nurses, and other clinical staff to validate and verify necessary corrections for accuracy and completeness.
  • Communicate with registration, IT, and clinical teams to resolve chart correct issues.  
  • Ensure all corrections are made in accordance with HIPAA, organizational policies, and regulatory standards.

Demonstrated computer aptitude and critical thinking skills required. Attention to detail.

Ability to make decision based on data and information obtained through research and analysis

Demonstrated ability to work independently with minimal supervision

Education:

High School Diploma or GED required.  Associates or Bachelor’s degree in Health Information Management (HIM) is preferred

AND

Experience:

Previous experience in Health Information Management or other health care field such as patient registration, unit clerk, nursing, etc.
