We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care. </p> <p>The Psychiatric Mental Health Nurse Practitioner Team Lead provides psychiatric care while supporting clinical leadership, care coordination, workflow optimization, and behavioral health program development. This role balances direct patient care with operational and administrative responsibilities, collaborating with IDTs and WelbeHealth PACE Physicians to deliver comprehensive, participant-centered care under the direction of the Medical Director.</p> <p><strong>This role is different because the Psychiatric Mental Health Nurse Practitioner</strong> <strong>Team Lead at WelbeHealth:</strong></p> <ul> <li>Serves a smaller patient caseload, providing the opportunity to form long-term relationships with participants and practice true value-based care</li> <li>Plays a critical role in supporting a growing, mission-driven healthcare organization, ensuring vulnerable seniors feel supported and have access to high-quality care</li> </ul> <p><strong>We care about our team members. That’s why we offer:</strong></p> <ul> <li>Medical insurance coverage (Medical, Dental, Vision)</li> <li>Work/life balance - We mean it! 17 days of personal time off (PTO), 12 holidays observed annually, and 6 sick days, CME days</li> <li>401K savings + match</li> <li>Comprehensive compensation package including base pay and bonus</li> <li>And additional benefits!</li> </ul> <p><strong>On the day-to-day, you will:</strong></p> <ul> <li>Provide psychiatric consultation, illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians, Community-Based PACE Physicians, and the IDT to develop a comprehensive care plan for each participant</li> <li>Collaborate with Physician and the IDT on discharge planning from acute or long-term care settings</li> <li>Partner with the PACE Physician or Community-Based PACE Physician to refer participants to a psychiatrist as indicated, as well as manage participant care in nursing homes, making regular visits as dictated by WelbeHealth standards and participant need</li> <li>Provide clinical leadership and mentorship to Psychiatric Nurse Practitioners and Behavioral Health team members</li> <li>Serve as the first point of contact for discussing complex cases and pre-enrollment decisions</li> <li>Support
