  • The PCU population consists of adult patients ages 18-64 and geriatric patients ages 65 and greater with a variety of medical – surgical conditions which may require frequent monitoring or treatment. A total of 19 beds are available on the East PCU.
    Scope and complexity of patient’s care needs: (i.e. most frequent diagnosis; admission/discharge criteria, if appropriate)
    The Progressive Care Units accommodate a wide variety of Medical and Surgical diagnoses, including patients with CVA. Admission is based on the need for frequent monitoring and intervention. Common interventions for these patients include central venous lines, cardiac monitoring, and continuous infusions of various medications.
    Methods used to assess and meet patient’s care needs: (includes services most frequently provided; policies/procedures; case management; team rounds; pathways)
    The Patient Focused Care model provides 24-hour comprehensive care including direct nursing care, patient and family education, and a variety of other services as needed. There is a collaborative delivery of services including; nursing, physician, occupational, physical, and speech therapy, social services, nutrition, pastoral care, respiratory therapy (RT); patient care resource manager (PCRM), and pharmacy. RT and pharmacy provide 24-hour service to the unit. Multidisciplinary rounds are completed routinely and include many of the above-mentioned services. Care of the patient/family is a collaborative effort by all team members and facilitated on a continual basis. The Clinical Nurse Specialist or Program Manager and the PCRM are available to assist with staff and patient/family education.
    Methods used to determine the appropriateness, clinical necessity and timeliness of support services provided directly or through referral: (How determined and evaluated?)
    Monitoring of the processes that address the important aspects of care (including infection control, airway patency, hemodynamic monitoring, and patient/family education) as well as patient outcomes is ongoing and a shared responsibility among all disciplines. Feedback from patients, families, physicians, and other health professionals is incorporated into planning for the support services required. Some other means of include: Emergency Department Evaluation, PCRM rounding, and Physician team rounding/assessment.
    Extent to which the level of care or service meets patient’s needs: (availability of necessary staff, list any contracted staff, hours of operation if other that 24 hours/day, 7 days/week and method used for ensuring hours of operation meet the needs of the patients to be served with regard to availability and timeliness)
    The East PCU unit is staffed by licensed professional registered nurses with advanced education in critical care nursing specific for the PCU. The PCU is operational 24 hours a day/7 days a week. The charge nurse receives a verbal report from each patient’s primary nurse, on a shift-by-shift basis. Direct care hours per patient day are used to guide staffing.
    Nurses, PCA’s, and CNS work collaboratively with other members of the Health care team to meet the needs of the patients and their significant others.
    The nurse manager maintains 24/7 responsibility of the unit activity with support from ANM, supply coordinator, administrative assistant and CNS.
