Here are a few of the daily responsibilities of our Home Care Nurse:
Client Care
1. If needed by state law, completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates client nursing needs.
3. Initiates the plan of care and makes necessary revisions as client status and needs change.
4. Develops a care plan and includes the client and the family in the planning process.
5. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the Florida Nurse Practice Act.
6. Counsels the client and family in meeting client needs.
7. Provides health care instructions to the client as appropriate per assessment and plan of care.
8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
- Current LPN or RN nursing license valid in the state of Florida
- Willingness to be available for any/all emergencies regarding the community
