Description:

• Under the direct supervision of the Cumberland Valley District, Jackson County Home Health team leader, administrator and/or their designee, employee will be responsible to:

  ·         Provide physical therapy services in accordance with the physician’s plan of treatment while maintaining accurate and timely submission of clinical documentation and records according to agency policy (60%)

  ·         Maintain communication with physical therapist and other disciplines (10%)

  ·         Consult with the physical therapist in the development of patient goals and progress notes (6%)

  ·         Keep updated on governing rules and regulations and agency policies and procedures (6%)

  ·         Participate with in-service programs and case conferences (5%)

  ·         Other duties as assigned by the supervisor or administrative staff (3%)
