What You'll Do:

• Perform home visits and develop personalized treatment plans
• Conduct diagnostic tests and assessments (mobility, range of motion, strength, balance, etc.)
• Implement therapeutic exercises, gait training, and balance interventions
• Educate clients and caregivers on rehabilitative care techniques
• Maintain accurate documentation using a state-of-the-art tablet-based EMR system
• Collaborate with nurses and other healthcare professionals to ensure comprehensive patient care

What We’re Looking For:

✔️ Current North Carolina Physical Therapist (PT) license
✔️ Strong interpersonal, organizational & problem-solving skills
✔️ Ability to work independently & manage time effectively
